Our office is located to the WEST of the hospital, just across from the west side hospital parking lot. If you are standing in front of the Lakeview Hospital main entrance, this building is to the right.
Both clinic locations are shared space with Oakes Family Practice, so you can also follow those signs.

Enter on the NORTH side near the emergency sign. There is a big “2” on the entrance door. If you are in front of the Lone Peak hospital main entrance, go the left side of the hospital building to find Building 2.
